The ARKRAY Discharge Program has partnered with Brookshire’s to help provide our patients with affordable test strips. Each pharmacy locator lists the nearest participating locations.
Complete the order form to have your stock of Discharge Kits processed and sent to the desired location.
*Please allow 5-7 business days for delivery. Prior to depletion, please place another order of kits to have on hand.
Note: Kits will come with the individual components, along with a bag to put the materials in. If you have a patient(s) that require a slightly larger meter, you can place a note in the comment section to request a small amount of GLUCOCARD Shine blood glucose monitors.
